During the course of the investigation, LPA conducted interviews with the Licensee (LS), four Staff (S1 – S4), 3 children (C3 - C5) and four adults (A2 – A5) from 10/31/2024 to 01/09/2025. LS denied the allegations. LS and Staff (S1 - S2, S4) stated that daycare children are never left unattended. LS and S1 stated that staff would position themselves to always have view of children when inside and outside the facility. Furthermore, LS and S2 stated a staff member would accompany children in a group when using the restroom. LS and Staff (S1 – S2, S4) stated that staff would count the children when transitioning them to the outside yard and inside area. S2 further stated the children would walk in a line with one teacher in the front and one in the back. A statement from S3 did state there was an incident when a child, from a different classroom from their own, was observed unsupervised and unsure if the child was left unattended, S3 stayed in view of the child till they returned to their classroom.
Additionally, LS and Staff (S1 – S4) stated there has not been an incident when a child wondered outside the facility. Statement from LS stated that C1 attempted to open the facility’s door but was always stop by staff member blocking the way. In addition, S3 stated that C2 liked opening doors but was never able to wonder outside the premise.
Statements from LS and staff (S1 – S4) indicated there are at least two staff in each classroom. LS and S1 further stated two staff are fully qualified teachers and Teacher Aides are never left alone with the daycare children. LS further stated a Teacher Aide is only alone with the children during naptime.
LS and Staff (S1 – S4) stated there is not anything hazardous accessible to daycare children. When completing any activity, staff would clean up the area. Additionally, from LPA’s observation on 10/21/2024 and 10/29/2024 no evidence of hazardous objects including wires were on the floor of the facility. Interviews conducted by Adults (A2 - A4) and Child 2 (C3 – C5) currently did not have any concerns with the allegations filed against the facility.
Based on available information, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore, the allegation is determined to be unsubstantiated at this time. There were no Title 22 deficiencies cited. This report was reviewed and discussed with the facility’s licensee, Zoe Teeter. Appeal rights were provided. Notice of Site Visit shall be posted for 30 days from today's visit. |
