A subsequent complaint investigation visit was conducted today by Licensing Program Analyst (LPA), Sebastian Phouthavong who met with Licensee, Zoe Teeter (LS) for the purpose of delivering complaint investigation finding for the above allegations. LPA, previously conducted an inspection on 02/19/2025 to initiate the investigation and met with Licensee to discuss the allegations, conduct interview(s), make observations, and request documents. LPA also conducted an inspection on 02/26/2025.It is alleged Staff do not provide appropriate napping equipment for the daycare and that Staff do not properly safeguard the facility grounds.

During the course of the investigation, LPA conducted interviews with Licensee (LS), Director (D1), two staff (S1 & S2), and 3 Adults (A2 – A4) from 02/19/2025 to 05/12/2025. LS stated that infants under 1 years olds sleep in a crib unless they can climb out of the crib which then are allowed to sleep on a cot. LS further stated 14 to 16 cribs are available and extra cribs and sheets if needed. Interviews from D1 and Staff (S1 & S2) stated children under one years old sleep in a cribs and that the facility has extra napping equipment for children corroborating with LS statement. Additionally, LS, D1 and staff (S1 & S2) stated the facility’s front door is always closed and locked from the outside. D1 and S1 stated individuals, including staff, must ring the facility’s doorbell for entry. D1 further stated if staff do not know the individual, staff would speak to them outside and ask for identification. LS and S1 did state the door is open when using the facility’s yellow cart to conduct walks around the area; but the children are under supervision when it occurs. S2 also state there are times when individuals would forget to close the door. Interviews conducted by Adults (A2 – A4) currently did not have any concerns with the allegations filed against the facility.

Based on the information gathered during this investigation, although the allegations may have happened or is valid, there is not a preponderance of evidence to prove that the allegations occurred and therefore are determined to be unsubstantiated. There were no Title 22 deficiencies cited. This report was reviewed and discussed with the Licensee, Zoe Teeter. Appeal rights were provided. Notice of Site Visit shall be posted for 30 days from today's visit.
