25 | On 08/25/2026, Licensing Program Analyst (LPA), Sebastian Phouthavong made an unannounced Case Management visit in response to a self-reported Unusual Incident Report (UIR) regarding a child being serviced milk when being alleged to milk. The UIR was received to Community Care Licensing on 08/17/2026. LPA met with Licensee, Zoe Teeter and spoke with her about the purpose of today's visit.
During today's inspection, LPA toured the facility inside and out and observed 61 children being supervised. LPA conducted interviews with Licensee, Assistant Director and one adult. Licensee, Assistant Director admitted that a staff member served milk to a child, not knowing the child had a milk allergy. Furthermore, Licensee, Assistant Director and one adult stated the child later needed medical treatment by medical professions due to an allergy reaction. In addition, Licensee, Assistant Director and one adult stated there was a previous incident regarding the child’s allergy. According to record review, it was indicates that the child has a milk allergy on file.
A notice of site visit was given and must remain posted for 30 days.
California Code of Regulations, Title 22, is being cited on the attached LIC 809-D. Appeal rights were provided. An exit interview was conducted, and this report was read and discussed with Licensee, Zoe Teeter. The Notice of Site Visit shall be posted for 30 days.
Reports citing Type A violation are to be provided to parents/guardians of children currently enrolled by the next business day or the next day the children are in care, and to parents/guardians of children newly enrolled at the facility for the next 12 months from the date of this report. Parents/guardians must sign Form LIC 9224 to be kept in each child's file.
