On 06/22/26 Licensing Program Analyst (LPA) Amy Strother made a Case Management visit to the facility to follow up on an Unusual Incident Report (UIR) received from facility representative, Monika Desor (S3) on 06/15/26. The UIR was regarding an incident that occurred on 06/12/26. On 06/16/26, S3 submitted a 7 minute 53 second, non-audio video recording from cameras at the facility, recorded on 06/12/26.

During today’s visit, LPA met with facility representative, Monika Desor (S3). At the time of LPA’s arrival there were two children in attendance, supervised by one adult, new Director, D1. Based on a review of records, although D1 had completed the LiveScan fingerprinting, D1’s fingerprints were currently listed as “In Process”. Based on interview, D1 began working at the facility with the children on 06/15/26 to date, with the exception of 06/19/26 when the facility was closed for the Juneteenth Holiday; working for a total of 5 days.

A third child arrived at the facility, for a total of three children in care. S3 called Licensee, Brij Desor to come in to take over the care and supervision of the children, and sent D1 home. S3 resumed the inspection visit with LPA Strother.


LPA requested a copy of the Child Care Roster. At 9:18am S3 stated that she did not have a roster or form LIC9040. LPA requested that S3 sit with LPA to review the video that had been submitted, asking S3 to identify the staff and children in the video. S3 identified 2 staff (S1 & S2) and 7 children (C1-C7), by name.

Based on LPA's observation of the video recorded on 06/12/26 staff 1 (S1) was observed on more than one occasion, handling children roughly. Time stamp 0:00:59 S1 is observed grabbing child (C3) by the left forearm, pulling C3 across the room by the forearm. C3 walks away from S1 holding their arms close to themselves.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

At time stamp 0:01:04 S1 is observed forcefully grabbing onto child (C4's) left upper arm, when C4 is observed getting into an open Bento Box style lunch left unattended at the child size table. When S1 let go of C4’s arm, C4 is observed recoiling, holding their arms close to their body, leaning away from S1.

At time stamp 0:04:59 S1 is observed using her left hand to push C4 out of a space on the side of the refrigerator.

At time stamp 0:05:04 S1 bends her knee, using her knee to nudge child C4 away from S1’s body.

At time stamp 0:05:26 C4 leans their upper body into S1’s legs, as if to get S1’s attention. S1 uses her foot and leg to scoot C4 away from her body.

At time stamp 0:07:31 child C2 is observed hitting child C1 on the left side of the forehead with a wooden block. During the remaining 22 seconds of video, no staff person approaches child C1. C1 does appear to be crying in the video, with their mouth wide open and their left hand held to the left side of their forehead.

S3 stated in the UIR no one consoled C1. LPA requested to review additional video footage to observe what transpired beyond the 22 seconds after C1 was hit in the head. S3 stated that no additional footage is available, as the footage is deleted after 7 days.

The following violations of the California Code of Regulations, Title 22; Division 12, were observed: see LIC 809D. Appeal Rights were provided. A Civil Penalty has been assessed in the amount of $500. See LIC421BG.

A notice of site visit was given to facility representative and must remain posted for 30 days.

Exit interview conducted and report was reviewed with facility representative, Monika Desor.

LPA Strother informed facility representative, Monika Desor that this report dated 06/22/26 documents 3 Type A citations. Type A citations which shall be posted for 30 consecutive days as there is/are immediate risks to the health, safety, or personal rights of children in care.

Also, LPA Strother informed the facility representative to provide a copy of this licensing report dated 06/22/26 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

During the inspection, LPA requested that S3 complete a roster to include all children that have been enrolled since the school was first licensed, listing the date left for children no longer enrolled. S3 provided LPA Strother with a completed roster by the end of the visit.

Based on interview at 9:18am, facility representative S3 stated that she does not have a roster of children in care or a completed form LIC9040, which poses a potential health, safety or personal rights risk to persons in care.
