At approximately 9:05 AM, Licensing Program Analyst (LPA) Robert Frank arrived unannounced to conduct a Required 1 Year inspection and was greeted by Administrator Gachu and staff member Darcy Harlan. Windsor House is Licensed as an Adult Residential Facility (ARF). The facility is a single story ranch house. The facility has an approved fire clearance for four (4) ambulatory Clients between the ages of eighteen (18) and fifty-nine (59). Upon arrival, LPA was informed that there were two (2) clients in care and two (2) staff members on-site. At approximately 9:20 AM, LPA reviewed the Facility's Staff Roster and observed that all staff on-site were background cleared and associated to the facility per regulation.

At approximately 9:30 AM, LPA toured the facility with Administrator Gachu. All exits were clear and unobstructed. All three (3) fire extinguishers were last serviced and tagged in 10/2024. Food supply was sufficient. The facility was sufficiently lighted with the exception of the kitchen area as the LED light was not working. LPA inspected two (2) client bedrooms and observed all to have sufficient lighting and furnishings as required per Title 22 Regulations. There was an appropriate supply of cleaning products, linens, hygiene products and paper products available for clients. Toxins were observed to be stored inaccessible to clients. Facility has an infection control plan as required. The facility has a required emergency disaster plan. The facility is conducting fire and emergency drills quarterly. The last disaster drill was conducted in 9/2025. The facility does have emergency food and supplies to meet the "72 hour shelter in place" requirements. Hot water temperatures for all sinks in facility were observed to be within Title 22 regulations of 105 to 120 degrees Fahrenheit. Facility smoke detectors and carbon monoxide detectors were tested and observed to be operational. LPA observed that window screens were not present in two of the facility’s front windows. One of these windows was open during the inspection. This deficiency will be cited.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

During the inspection. LPA observed the following: an extremely strong mildew/mold odor emanated from the front entrance area and in the closet next to the front door. There was a damaged drawer in the kitchen with exposed nails. The area under the kitchen sink was observed to have mold and mildew. There was a large hole in the wall directly above the refrigerator. The cabinet above the refrigerator was damaged. There was a broken window next to the rear door in the kitchen area. The ceiling in the front dining area was peeled back from water damage. There were full trash bags lying on the ground on the side of the house. There was various debris throughout the yard area. The bathtub caulking was peeling and was covered in mildew. These deficiencies will be cited on complaint 21-AS-20250905085117.


At approximately 11:00 AM, LPA reviewed two (2) client files. Client files were observed with all required documentation. The facility has four (4) Staff Members. LPA reviewed three (3) staff files. One (1) of three (3) staff files were observed with all required documentation including First Aid and CPR certification. Two (2) of three (3) staff files for staff members S1 and S2 were observed not to have an LIC 503 Medical Assessment and Tuberculosis test results. Staff Member S4 was observed not to have a staff file at the facility. These deficiencies will be cited. Staff member S4 not having a valid first aid certification will be cited on complaint 21-AS-20250905085117.

LPA spot checked Medication for two (2) clients. LPA observed that there was no Centrally Stored Medication and Destruction Record for client C2's prescription medication. LPA further observed that client C1's Centrally Stored Medication and Destruction Record did not have the expiration date listed for the prescription medication. These deficiencies will be cited on complaint 21-AS-20250905085117. The Facility does not maintain client’s monies for personal and incidental items.

The Facility does not have a working phone. This deficiency will be cited.

Marylis Gacho's Administrator Certification 6076296735 is current with an expiration date of 5/28/2027.
