Licensing Program Analyst (LPA) Willis arrived unannounced to conduct a Case Management inspection and met with Executive Directior Tracey Mease and Resident Services Director, Tru Coiner

During complaint investigation LPA discovered a violation of regulation unrelated to the current complaint investigations. Resident, R1 fell in the facility on March 31, 2021 and was subsequently transported to the hospital where they were diagnosed with a fracture.

CCL has not received a notification of the fall and injury to date which is required by regulation.

Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Appeal rights given. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties.
