25 | Licensing Program Analysts (LPA) Victoria Willis and Erik Gonzalez Campos arrived unannounced to conduct a case management visit. LPAs toured memory care and assisted living and observed multiple areas of concern.
In memory care unit LPAs observed a black and leaking hole in the ceiling of an unoccupied room, approximately a foot in diameter. Facility currently has construction on the south end of facility that is slated for additional parking. Area is partially taped off with caution tape. Hazards include two excavators, exposed pipe, and uneven ground including but not limited to a large hole where the pipe is and an abrupt drop on one side of the perimeter.
LPAs also observed carpet in resident room to appear stained with a large red splotch and multiple areas of general uncleanliness.
Civil penalty in the amount of $250 is being assessed because facility violated the same regulation within a a 12 month period.
Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Appeal rights given. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. |
