25 | Licensing Program Analyst Willis arrived unannounced to conduct a case management inspection and met with facility representative, Cinthya Alvarez (Gamino).
LPA visited facility on April 15, 2022 and was provided four incident reports for residents. Three out of four reports were not submitted to CCL within the seven day time frame required by regulation. More recently, LPA learned about another incident that was not reported to CCL timely. LPA discussed reporting requirements with facility representative and provided the regulation for further review.
Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Appeal rights given. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. |
