25 | LPA Cuadra arrived unannounced to conduct a case management inspection was greeted by staff, Elsie Haynes and Administrator, Hanna Peralta arrived later; Nayeli Hidalgo RN consultant from North Bay Regional Center was also present at the time of visit. LPA conducted risk assessment with staff prior to the visit. LPA arrived at the facility and had her temperature checked and was logged into a sign-in sheet.
LPA is following up on an incident received on 11/18/21 from Administrator regarding client (C1). On 11/10/21 C1 was scheduled for a DTAP immunization appointment. Facility staff transported C1 to Santa Rosa Community Health to see their Physician who ordered two immunization shots (Tdap and flu). At around 5:30pm the facility received a call from the clinic notifying Administrator that C1 received two immunization shots that day and that C1 already received a dose of flu shot (Fluzone 0.5ml) on 10/26/21. Upon receiving call staff informed licensed nurse and staff on duty about the incident and C1 was closely monitored, their vitals were checked for any adverse reaction and their responsible parties were notified. Facility implemented a preventive plan that includes a communication log with specific instructions for staff to track client's appointments and immunization.
During today’s visit LPA reviewed C1’s file including facility communication log that includes the documentation of two doses of flu vaccine administered by Santa Rosa Community Health on 10/26/21 and 11/10/21. Facility also provided a copy of records for LPA to review.
No deficiencies were cited during today's inspection. |
