On 2/13/25 LPA delivered complaint findings for complaint 21-AS-20240906084230 and issued citations for deficiencies of regulations: 80078(a) and 85064(e) with plans of correction for both deficiencies due on 2/14/25. As of today 2/21/25, CCL has not received a plan of correction for deficiencies of regulations: 80078(a) and 85064(e). Therefore, CCL is issuing a civil penalty for failure to correct deficiencies of regulations 80078(a) and 85064(e) for the period of 2/15/25 through 2/21/2025 at $100 per day for each respective deficiency for a total of $700 for each respective deficiency.

The findings were changed from Substantiated to Unsubstantiated due to the residents actually making it their appointments, by way of caregivers' personal vehicles. Additionally, the related 9099D issued was also amended to remove deficiency 80074(c) that was issued as a result of the substantiated complaint finding for allegation "Staff are not transporting residents to and from appointments" alleged in complaint 21-AS-20240906084230.**
