25 | Licensing Program Analyst (LPA) Hansen arrived unannounced to conduct a Case Management visit due to repeat deficiencies that were observed during a Complaint Investigation.
Licensing Program Analyst (LPA) Hansen observed facility uses Medication Administration Records (MARS) to document medication records. Facility uses a MAR which documents each medication dose that a resident is given by staff initializing the field when medication is given. Facility was cited at a case management 12/22/2022 from a Complaint investigation for failing to always complete all MARS refusals and did not note reason for medications not being administered, including but not limited to resident refusals. Deficiencies cited Resident Records 87506(a).
While conducting another complaint investigation 21-AS-20221102092137 LPA reviewed records and found resident (R1)’s medication records incomplete. LPA was informed by Resident Care Coordinator, Samira Howeidy facility was unable to locate any written or electronically completed medication records for the entire month of October 2022. On today’s case management the facility is receiving Civil Penalties for a repeat violation of Resident Records 87506(a).See LIC809-D.
Administrator was informed Regional Office will be requesting an Informal Meeting to discuss areas of concern identified.
**Civil Penalty are issued today in the amount of $250 for a repeated violation within a 12 month period.
Appeal of Rights Given.
The following deficiencies were observed (see LIC 809D) and cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. Exit interview conducted and appeal of rights provided. |
