25 | Unannounced annual visit made out to this day program on 11/18/2021 by LPA Charlie Yang who was met by the Director of Operations, Jesus Munoz, and Executive Assistant Heather Woodard. This LPA requested that facility personnel go ahead and contact the facility designated Administrator, Carla Strong, but was informed that she was out of the day program at this time and would not be available for today's visit.
It was learned that this program has been closed to in person learning since early 2020 and has not had any clients present since then. All classroom and programs have been conducted via virtual alternatives for all clients at this time.
Brief interview conducted with facility personnel who were present. Tour of this day program was conducted.
This day program consisted of two buildings which were located adjacent to each other. Each building housed a different type of program and different type of clients as well. Classrooms were toured in both buildings. Furniture and furnishings were observed to be sufficient at this time. This day program was in the process of cleaning and setting up some of the classrooms to be in compliance with COVID 19 policies and procedures.
Day program restrooms were toured. Hot water temperatures were taken and measured to make sure that they were within the allowed range of 105-120 degrees.
Fire extinguishers were observed to have been annually inspected on 04/14/2021 by the local fire extinguisher company and in compliance at this time.
Exterior grounds of the buildings were toured. Perimeter fence, side gates, and exterior areas intended for client use were toured and observed to be in compliance at this time.

LIC 308, LIC 400, LIC 500, and LIC 610
There were no deficiencies observed or cited during today's annual visit.
