| The following documentation is required to proceed with the removal of the Toddler Program:
1) Updated facility application to reflect the preschool capacity of 113.
2) Board resolution reflecting capacity change.
3) Change in capacity fee of $25.00.
4) Request for Fire Clearance Form.
The items above are required to move forward with the licensee’s request to remove the toddler option from the license. Licensee has already provided an updated facility sketch, Emergency Disaster Plan, Personnel Report, and Administration Organization Form.
This facility does provide care to infants. LPM Brannon and LPA Mendoza discussed the safe sleep regulations with Center Supervisor Yasmin Gallegos and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource.
LPM Brannon and LPA Mendoza also informed Center Supervisor Yasmin Gallegos, of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Exit interview conducted and report was reviewed with Center Supervisor Yasmin Gallegos.
A copy of this report is to remain in the facility for public review.
