25 | On 09/18/2024, Licensing Program Analyst (LPA) Xona Xayavong and Licensing Program Manager (LPM) Kari McWilliams arrived at the facility to conduct a Case-Management-Incident inspection. LPA and LPM met with Director Heidi Nunes and ECE Coordinator Kimbra Houck. LPA and LPM explained that the reason for the visit was to account for an unusual incident.
An unusual incident report was submitted to the Fresno Regional Office regarding an incident that occurred on 08/14/2024, where Parent #1 (P1) reported to the facility that staff #1 (S1) caused bruises on child #1 (C1) wrist and arm. P1 reported that during the first day of school at pick up time C1 was observed crying and when P1 observed C1 try to come to P1, S1 was observed pulling C1 toward them. It was reported that C1 had bruising from the incident.
During today’s inspection Director Nunes and ECE Lead Houck provided LPA Xayavong and LPM McWilliams with the following items: P1 complaint, S1 and S2 Statement of Facts, pictures of C1 wrist and arm, Incident - Follow-Up, and video footage from security camera on site. Also, Licensee provided Summary of Investigative Findings that was conducted internally. It was also confirmed that C1 returned to the facility and guardians of C1 have filled out paperwork to be volunteers in the class and there have been no issues.
Based on the observations of the videos, pictures and statements provided to LPA Xayavong and LPM McWilliams were able to determine that the facility met the child’s health and safety and personal rights. LPA Xayavong and LPM McWilliams observed no bruises from the pictures and C1 appeared to be in happy spirit when walking out of the classroom with P2. Also, P2 was holding the same arm when walking C1 out of the classroom.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.
