25 | On 08/19/2024 Licensing Program Analyst (LPA) Xona Xayavong and Licensing Program Manager (LPM) Kari McWilliams arrived at the facility to conduct a Case Management-Incident inspection. LPA and LPM met with Director Heidi Nunes and ECE Coordinator Kimbra Houck. LPA and LPM explained that the reason for the visit was to follow up on an unusual incident report. LPA Xayavong and LPM McWilliams toured the facility and a census was taken.
On 09/09/2024, Fresno Regional Office received an unusual incident report stating on 09/03/2024 at approximately 10:55 AM, during pick-up time, Grandparent found child #1 (C1), outside near another parent unsupervised by staff. Staff #1 (S1) saw C1 running up to his Grandparent and called for C1 to come back to the classroom. Approximately at 11:10 AM, Grandparent reported to the facility Administration that their grandchild was left on the playground without staff supervision. The facility Administration is conducting an ongoing internal investigation regarding the incident which will include interview, statements, and video footage review. A meeting will be conducted with S1 to review the supervision, reporting policies, and proactive steps to take to ensure the safety of all children.
During today’s inspection, licensee provided statements from staff, outline of the events during internal investigation, and video footage around the facility. LPA Xayavong and LPM McWilliams observed on the video footage during pick-up time C1 left the classroom and walked across the playground toward the exit gate. After approximately four (4) minutes C1 is seen walking behind another parent back to the classroom and the Grandparent walking toward the classroom shortly after. LPA Xayavong and LPM McWilliams observed staff were unaware that the child left the classroom as child walked away without being stopped by staff. LPM found that the facility did not comply with ensuring that children have 100% supervision while in care. Staff did not follow policy and procedure that children were supervised at all times.
