Licensing Program Analyst (LPA) Jason Lund conducted an unannounced visit to deliver findings for a complaint investigation. LPA met with Tanisha McNeal and explained the purpose of today's visit.

The investigation included, reviewing resident files and staff records. LPA also interviewed care home staff, witnesses and clients.

During the course of the investigation, LPA conducted interviews with staff and witness and clients. Client (C1) Valley Mountain Regional Center Individual Program Plan (IPP) dated 5/3/2019 states that C1 will continue to monitor C1’s anger and frustration. When Client (C1) gets flustered C1 will stutter. C1 believed that a staff member of the facility mocked C1 when C1 was upset. During the interviews process of the investigation C1 admitted staff might have been trying to ask C1 a question or trying to calm C1 down when C1 gets upset. LPA interviewed staff, would have reported to Administrator Tanisha McNeal if a staff member would have mocked C1 or any other clients.

The allegation indicates that the facility didn’t have adequate food at the facility. It was reported that the facility didn’t have enough food and the food was not fresh. LPA Lund was at the facility on 6/16/2021 and the facility did have two-day perishable and seven-day non-perishable food supply on hand. LPA interviewed staff, witnesses and clients and all stated the facility has enough food on hand.


Based on interviews of staff, clients and witness there was no inappropriate forms of discipline to the clients in care at the facility reported to the Administrator. It was reported that the facility staff would send clients to their rooms and was told they could not come out their room until told. The administrator Tanisha McNeal stated that the clients are free to go through out facility.

Based on interviews of staff, clients and witness there are no restrictions on client’s use of the facility phone. The clients are aloud to use the house phone throughout the day. The facility was getting told that when someone would call the facility often no one would answer. The facility did tell clients families if they could wait until the afternoon to call the clients it would be better. The facility would not stop any client or client family from using or calling the facility.


Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
