On 2/9/23, Licensing Program Analyst (LPA) Joseph Pacheco conducted an unannounced complaint inspection. LPA met with Director, Meredith Lawrence. The purpose of the inspection was to deliver the findings for the above complaint allegation.

During the course of the investigation, LPA interviewed Complainant, Director, and day care staff. Based on the information obtained during the investigation, there is a preponderance of the evidence to prove staff are not following proper COVID-19 guidance; therefore, the allegation is substantiated.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page, 809 D). Licensee was provided a copy of their appeal rights.

LPA Joseph Pacheco informed Director, Meredith Lawrence that this report dated 2/9/23 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Joseph Pacheco informed the Director, Meredith Lawrence to provide a copy of this licensing report dated 2/9/23 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview conducted and report was reviewed with Director, Meredith Lawrence.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Director is to review the most current California Department of Public Health guidelines and Stanislaus County Public Health guidelines and submit a mitigation plan on how the facility will protect children in care, staff and the community from COVID-19. Plan to be submitted by 2/10/23.
