On 8/29/2024 at 11:30 am, Licensing Program Analyst (LPA), Joseph Pacheco conducted an announced Pre-Licensing inspection for a new license. Upon arrival, LPA met with Facility Representative, Staci Coelho. Facility is located at 1225 Cooper Ave. Turlock, CA 95380 inside Sacred Heart Elementary School. Applicant is Pastor of Sacred Heart Church of Turlock. Applicant is requesting to be licensed for 23 preschoolers ages 2-6 in classroom D. Hours of operation will be Monday – Friday 7:00 am – 5:00 pm. This facility will operate on a traditional school year schedule. Facility is requesting a waiver to allow for Staci Coelho, currently the Director at Sacred Heart Preschool, (500311278 and pending change of ownership 503810270) to be the Director of this facility.
All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Representative stated that they will be in a locked cabinet behind the teacher’s desk inside the classroom. A fully equipped first aid kit is located in the classroom behind the teacher’s desk. There is an operational carbon monoxide detector on site located in the classroom. All required licensing documents were observed posted in the classroom. Children will be signed in and out electronically via a phone app or parents can also use a computer tablet supplied by the facility.
LPA continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured to an approximate 828 square feet which is sufficient to accommodate the requested overall capacity of 23 children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots/mats, bookshelves, and other activity supplies for the children. Drinking water is available from a water vendor service and is used to fill pitchers in the classroom. Children bring water bottles from home to drink from, but the facility has paper cups on hand or bottled water in case a parent forgets to send their child with one. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted by the Turlock Fire Department on 5/29/24.
Facility is requesting a waiver to have preschool children share a boys and girls bathroom located at the end of the northern hallway on the west side of the building with elementary school children. LPA observed a total of one toilet, 2 urinals and 2 sinks in the boys bathroom and 3 toilets and 2 sinks in the girls bathroom available for children’s use. These are sufficient to accommodate the requested overall capacity of 23 preschool children. Representative understands that staff are to escort children to and from the bathrooms and ensure there is no commingling of preschool and elementary school children while using the bathroom. Facility also has access to a bathroom located in the school gym next door to Classroom D that they can use in an emergency which Representative states is not used by elementary school children or staff during hours of operation. LPA observed 2 toilets and one sink in the girls gym bathroom and one toilet, one urinal and one sink in the boys gym bathroom. The isolation area for children who are ill will be in the staff area of the classroom and they will use the staff bathroom down the eastern hallway.
Facility will provide breakfast, lunch, AM snack and PM snack. Food is prepared off-site at the Sacred Heart Preschool kitchen located at 1250 Cooper Ave. and delivered to the facility daily.

Facility is requesting a waiver to allow for children to use the large outdoor activity space located on the east side of the premises of Sacred Heart Preschool at 1250 Cooper Ave at separate times as they do not have their own exclusive outdoor activity area on the premises. Children will have access to the playground via a street that is fenced off and not open to the public during hours of operation. The children in Classroom D will not have access to the small playground at Sacred Heart Preschool at 1250 Cooper Ave. located on the west side of the building that is separately fenced from the playground on the east side of the building. Chain link and wrought iron fencing on the east playground are at least four feet high. The total square footage for all the outdoor activity space for the preschool measured to an approximate 4,502 square feet which is sufficient to accommodate the requested capacity of 23 preschool children. Shade in the playground area is provided via a shade tarp over the high climbing equipment. Facility also has a shade structure in a separate area of the yard. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing structure on the playground which is properly anchored. There is adequate cushioning in fall zones of climber provided by rubber cushioning. Staff bring water pitchers filled with water during outside activities which is used to fill children’s water bottles they bring from home. LPA observed all hazardous items on the playground(s) to be inaccessible to children. Director was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA reviewed with Director the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Director was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Exit interview conducted and report was reviewed with Representative, Staci Coelho.
