On 2/25/25 at 9:00 am, Licensing Program Analysts (LPA’s), Joseph Pacheco and Pa Kou Vue conducted an announced Pre-Licensing inspection for a Single License conversion. Upon arrival, LPA met with Director, Ariana Sederquist. Facility is currently licensed for 108 preschool children (500301324). Applicant is Monte Vista Chapel, Inc. and they’re requesting to be licensed for 72 preschoolers ages 2-6 in classrooms 112-117 in the NEB building; 24 toddlers ages 18 months – 36 months in classrooms 131-132 in the CMB building; 16 infants ages 0-18 months in classroom 130 in the CMB building; and 14 school age children ages 4 years, 9 months – 7 in classroom 110 in the NEB building. This facility will operate year round. Hours of operation will be Monday – Friday, 7:00 am – 6:00 pm. During the times of year when school is in session, the school age component will operate Monday – Friday, 11:30 am – 6:00 pm.
All indoor and outdoor activity space utilized for the children was inspected today. LPA’s informed Director that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Director stated that they will be in Classroom 111 which is a staff office space. A fully equipped first aid kit is located in Classroom 111 in the NEB building on the south wall and on the south wall of the infant nursery reception area in the CMB building. There is an operational carbon monoxide detector on site located in each classroom and in the hallways of each building. All required licensing documents were observed posted in the hallway of the NEB building and in the lobby of the CMB building. Children will be signed in and out at through an electronic app but the facility does have a back up for handwritten signatures in the event the electronic app is not working.
LPA’s continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space for the preschool component measured to an approximate 3,206 square feet which is sufficient to accommodate the requested capacity of 72 preschool children. Total indoor activity space for the infant component measured to an approximate 698 square feet which is sufficient to accommodate the requested capacity of 16 infant children. Total indoor activity space for the toddler component measured to an approximate 1,335 square feet which is sufficient to accommodate the requested capacity of 24 toddler children.

Total indoor activity space for the school age component measured to an approximate 623 square feet which is sufficient to accommodate the requested capacity of 14 school age children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots/mats, cribs, bookshelves, and other activity supplies for the children. Infant toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water is available in the classrooms via drinking fountains. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance for 16 infant children ages 0-18 months, 24 toddler children ages 18 months – 36 months, 72 preschool children ages 2-6 and 14 school age children ages 4 years 9 months to 7 was granted on 2/12/25 by the Turlock Fire Department.
In the infant room high chairs and feeding tables have broad-based legs, plastic seats are in good repair and trays lock onto chairs. High chairs are made of washable, moisture-resistant material. Changing tables have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. Crib area is separated from activity space via being in a physically separate adjacent room to the infant classroom that can be shut with a door. Director understands staff are to remain in the crib room at all times when an infant is sleeping. Sleep logs are kept in the child’s file and in the infant and toddler classrooms in the CMB building. Needs and Services Plans and Sleep Plans are kept in in the infant and toddler classrooms in the CMB building and in the child's file.
LPA’s observed a total of 6 sinks and 6 toilets available for use in the preschool area which is sufficient to accommodate the requested capacity of 72 preschool children. LPA’s observed a total of 4 sinks and 4 toilets available for use in the toddler area which is sufficient to accommodate the requested capacity of 24 toddler children. LPA’s observed a total of 1 sink in the school age area. Although two toilets were observed in the school age bathroom, the facility does not have the ability to install privacy partitions between both toilets due to the size of the bathroom. There is a door to the bathroom that can be shut to allow school age children privacy from the classroom. Because of this set up, facility understands that only one school age child can use the bathroom at a time. LPA observed a total of 2 sinks and 2 toilets available for use in the infant area which is sufficient to accommodate the requested capacity of 16 infant children. Potty training is not currently being provided for infants. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be room 111 and they will utilize the staff bathroom located between rooms 130 and 131 or outside the NEB building. Facility will provide AM and PM snacks for all components. Parents will provide lunch for all components.

Facility has a supply of food on hand in case a parent forgets to send their child to school without a lunch.
The kitchen area currently includes a refrigerator, freezer and sink and is located in Room 111 in the NEB building. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children via being in a physically separate area from the classrooms.
The facility currently has two fully fenced playground areas and has submitted a waiver request to allow the infant and toddler components to share one of the outdoor activity areas at separate times. The infant and toddler component outdoor activity space is adjacent to the CMB building. Facility has also submitted a waiver request to allow the preschool and school age components to share the other outdoor activity area at separate times which is adjacent to the NEB building. Wrought iron fencing in the infant and toddler outdoor area is at least four feet high. LPA’s observed a section of wrought iron fencing in the preschool and school age outdoor area that was not four feet high. The total square footage for the outdoor activity space for the preschool and school age outdoor activity area measured to an approximate 4,877 square which is only enough space to accommodate a maximum of 65 children at one time. The total square footage for the outdoor activity space for the infants and toddlers measured to an approximate 3,302 square feet which is sufficient to accommodate the requested capacity of 16 infant children and 24 toddler children at separate times. Shade is provided via shade canopies in both areas. Mature trees provide additional shade areas. There are sufficient outdoor age-appropriate toys and play equipment available on the playground(s). No high climbing play equipment was observed in the infant and toddler outdoor area. There is a climbing structure on the preschool and school age playground for children ages 2-5 according to the manufacturers sticker which is properly anchored. There is adequate cushioning in fall zones of climber provided by rubber chip cushioning. Drinking water is available via a water fountain in the preschool and school age outdoor area. In the infant and toddler outdoor area, LPA’s observed an igloo jug with a paper cup dispenser. LPA observed all hazardous items on the playground(s) to be inaccessible to children. Director was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a

LPAs reviewed with Director the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

LPAs discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPAs also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Director was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The following corrections are needed prior to the issuance of the license:

Indoor
· Make modifications to water fountain so that it’s not a drowning hazard or make inaccessible to children in care.
· Obtain a trash can with a tight-fitting lid for solid waste disposal.

Outdoor
· Modify fencing along southwest corner of the preschool and school age playground so that it’s four feet high.

Director, Ariana Sederquist understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.

A return inspection will be needed to verify corrections/modifications have been made. Pending a final file review, completion of the above items and approval of the outdoor waivers by the Fresno Regional Office. A license for 72 preschoolers, 24 toddlers, 16 infants, and 14 school age children will be granted.

Exit interview conducted and report was reviewed with Director, Ariana Sederquist.

A notice of site visit was given to Director and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. No deficiencies were cited.
