On 01/25/2022, Licensing Program Analyst (LPA), Luisa Gavoutian, conducted an unannounced case management inspection. This inspection is a continuation of the annual inspection conducted on 12/15/2021. LPA was greeted by Licensee Rashel Baghdasarian. Also present were two fingerprint-cleared live-in adult relatives. Present during today’s inspection were five children.

Adequate supervision is being provided during this inspection, including to napping infants. Licensee has two infants in care. Licensee stated that she has been doing the 15-minute logs each day but does not keep them and throws them away at the end of the day. LPA informed Licensee that she is required to maintain the documentation in the children's files. LPA observed that Licensee was checking on sleeping infants every 15 minutes during today's inspection. This facility is following appropriate COVID-19 safety guidelines as set forth by the California Department of Public Health for child care providers. LPA observed adults and children over age two wearing face coverings. Fire drills are conducted and documented at least once every six months. A current roster of children is maintained.

There is a working telephone and cellphone number was verified. LPA reviewed a sample of children’s files, which were all complete with emergency information as required. LPA observed Child 1 was under the age of 12 months and Licensee did not maintain the Individual Infant Sleeping Plan (LIC 9227) in the infant's file. Licensee maintains documentation of immunizations for influenza, pertussis, and measles for herself and staff. Licensee’s Pediatric CPR/First Aid are current expiring on 06/14/2022. Licensee’s Mandated Reporter training certificate was completed on 10/15/2020. Licensee is aware that any authorized employee of the Department may enter and inspect any place providing personal care and services at any time, with or without advanced notice. Days and hours of operation are Monday – Friday; 7:00 a.m. – 6:00 p.m. (Continued on LIC 809-C)

LPA & Licensee discussed the Community Care Licensing (CCL) website (www.ccld.ca.gov) which provides access to Provider Information Notices (PINs), Quarterly Updates, Mandated Reporter Training, Forms, and Regulations. LPA provided Licensee with the “Effects of Lead Exposure” brochure in accordance with AB 2370, Chapter 676, Statutes of 2018.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department.
