· 102425(c) Licensee provided 15-minute Safe Sleep logs for infants in care; however, is still missing the LIC9227 Form for infants (Child) #1, #2 and #3. Licensee was given a Plan of Correction date of 03/21/2024. This deficiency was due to be corrected by March 21, 2024. Licensee has failed to correct this deficiency, therefore a $100.00 per day civil penalty is assessed starting, March 21, 2024, through March 27, 2024, and will continue at $100.00 per day until corrected. Licensee will need to inform the department once correction is complete. See LIC 421 for civil penalty assessed.
During the record review of children’s files, Licensee revealed that Child #2 and #3 did not have their own files. This poses a potential risk to the health, safety, or personal rights of children in care.
LPA conducted an inspection of the built-in swimming pool in the backyard, which is now secured with a 5' mesh fence and made inaccessible. The pool gate is self-latching, self-closing and opens away from the swimming pool. No windows or doors have direct access to the pool area. LPA observed a bicycle missing rubber handles revealing metal, which could cause injury. This poses a potential risk to the health, safety, or personal rights of children in care.
LPA cleared deficiencies the above on this date and provided licensee with a "Letter of Deficiency Citations Cleared." This letter must be filed in the facility for three years and upon request made accessible to the public for review. LPA provided Licensee with an Informal Meeting Letter, requesting Licensee to attend at Fresno Regional Office on 04/11/2024 at 11:00 a.m. Licensee acknowledges responsibility in attending such meeting and reassured LPA she would be present.
Per Chapter 3, Division 12, Title 22 of the California Code of Regulations, the following deficiencies were cited during today’s visit. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. Appeal Rights provided. |
