On 02/24/2026, Licensing Program Analysts (LPAs) Valerie Mireles and Sha Fee Vang conducted an unannounced complaint inspection. The purpose of the inspection was to open and close an investigation into the above allegation. LPAs met with Licensee Tara Weber. Also present was Licensee's assistant. LPAs explained the allegations, toured the facility, inside, outside and a census was taken.

This agency investigated the complaint alleging Licensee does not ensure home is maintained free from conditions which might endanger a child. Licensee reported that the day care room and hallway bathroom are used for providing care and Licensee or her assistant escort the children from the daycare room to the hallway bathroom. During the investigation and facility tour, LPAs observed medications, alcoholic beverages, cleaning wipes, expired food and excessive clutter in areas not used for the daycare, but are accessible to the children in care. Children need to be in a safe, healthful, and comfortable environment. This poses a potential health and safety risk to children in care. Based on LPAs observations and interviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.
