LPA Galvan attempted to visit this facility for a 90-day visit with provider. Upon entry to the home, LPA Galvan inquired as to whether anyone in the home had recently tested for Covid-19 and/or was awaiting results. Licensee replied that yes, her 3 children had taken at-home tests on 1/8/2022 and were showing POSITIVE. LPA Galvan immediately went back outside of the house and spoke with Lenet through the iron security door following social distancing protocol, wearing a tight-fitting mask and being at least 6 feet away.

Licensee states she did not contact the licensing office because she did not know she had to. Licensee stated she does not have any other children besides her own in the home at this time. She stated she is having a hard time finding children to care for. LPA Galvan informed Lenet that she should be reporting any and all Covid-19 positives to both licensing office and the Department of Public Health. LPA Galvan gave the number for Stanislaus County Dept of Public Health to licensee and advised her to call and report her children's results immediately. LPA Galvan also informed licensee that she was to submit a LIC624 Unusual Incident Report within 7 days. LPA got information that licensee's three children, ages, 8,5 and 2 years of age are Positive for Covid-19, Lenet states she has notified her children's school and they advised her not to send the kids to school for 10-days. Licensee states she is not sending them to school and has her facility "closed" for 10 days. Licensee states her oldest child will return to school on 1/18/2022 if he has no symptoms.
Licensee did inquire about not wanting her license due to her not being able to recruit any children to her facility. LPA advised Lenet to consider going inactive instead. LIC9211 Inactive Request form was emailed to bodygardno@hotmail.com.

On 1/18/2022 LPA Galvan contacted Lenet at number listed 209-585-8257 to inquire as to whether she ever contacted Stanislaus Co. Dept of Public Health, Lenet said she had not, she just wanted to make her license Inactive immediately. LPA advised licensee to send LIC9211 back immediately so that would be arranged.
