25 | On 04/20/2023, Licensing Program Analysts (LPAs), Jeovanna Yanez and Paul Garcia met with Applicant, Brianna Ornelas for a pre-licensing/change of location inspection. Applicant, her husband, her adult roommate and four minor children reside in the home. Background clearances are discussed and LIS 531 is signed indicating that the adults currently living in the home and/or providing care and supervision to children have a criminal record clearance or exemption.
Facility was inspected inside and outside as shown on the facility sketch and the following items were discussed:
· This is a single story three bedroom, two bathroom home and children will have access to the living room, kitchen, dining room, bedroom #3 and hallway bathroom. Off-limits rooms are made inaccessible by use of plastic door knob covers. Applicant will escort children to and from the kitchen.
· There is a fireplace in the living room that applicant states will not be used during day-care hours.
· In the living room, LPA observed a flat screen television mounted on the wall, comfortable furnishings, children size furniture and table, and safe games, toys, and books for the children. In bedroom #3, LPA observed a bunk bed that will not be used for daycare, one crib, and one play yard. Children will nap on cots, infants will nap on play yards in bedroom #3. Applicant understands she is to supervise children at all times. LPA provided applicant with Individual Sleeping Plan and Safe Sleep handout.
· Facility has 3A40BC fire extinguisher, smoke alarm, carbon monoxide alarm and first aid kit in place.
· Applicant’s Pediatric CPR and First Aid certification was completed through HSI Pediatric with Emergency Medical Services Authority stickers (EMSA) and expires on October 18, 2024.
· Preventative Health and Safety Course was completed on March 17, 2019. Certificate of Lead Poisoning Prevention was completed on August 5, 2021. (CONTINUED OF 809-C) |
