32 | · Applicant’s Pediatric CPR and First Aid certification was completed through HSI Pediatric with Emergency Medical Services Authority stickers (EMSA) and expires on January 21, 2025.
· Preventative Health and Safety Course with Prevention of Lead exposure certification was completed on February 4, 2023.
· Applicant completed the Mandated Reporter General Training on January 26, 2023.
· SB 792 immunizations verified and on file.
· Applicant rents/leases the home, proof of landlord notification is required. LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. Applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).
· In the daycare room, LPA observed a flat screen television mounted on the wall, learning wall art, children size furniture, table and chairs, wooden cubbies for storage, a small trampoline, one crib, one high chair, and safe games, toys, and books for the children. Children will nap on mats, infants will nap in cribs. Applicant understands she is to supervise children at all times.
· Fenced backyard has cement and sodded area for the children. There is a patio porch for shade. LPA observed safe toys for the daycare children.
· Facility has 2A10BC fire extinguisher, smoke alarm, carbon monoxide alarm and first aid kit in place.
· Knives and medications are stored in the main part of the home. Cleaning compounds are stored in a storage closet made inaccessible via safety latch.
· There is an in-ground pool that is fenced with 5 ft. black mesh fencing, in accordance with Title 22 Regulations. The gate is self-latching/self-closing, swings away from the pool and latching device is located no more than six inches from the top of the gate.
· There is a dog that is kept in an off-limits room. Applicant is advised it is her responsibility to ensure the safety of children in care at all times from the pets.
· Firearms and ammunition are locked and stored separately in accordance with Title 22 Regulations.
· Advised applicant fire drills are to be conducted once every 6 months and must be documented with date and time. A fire drill log was provided as an example.
