On 05/12/2026, Licensing Program Analysts (LPAs), Aurelio Mendoza and Jose Ruiz, met with Applicant, Tiffany Horsley, for a change of location pre-licensing inspection. Applicant resides in the home with her husband and two minor children. Criminal record clearances were discussed, and LIC 531 was signed, indicating that all adults residing in the home and/or providing care and supervision to children have obtained a criminal record clearance. Fire clearance was granted on 04/27/2026.

The facility was inspected inside and outside, as shown on the facility sketch, and the following items were discussed. Fire clearance was received on 05/05/2026. A fire pull alarm is located on the right-hand side wall of the home’s entryway.

This is a single-story, four-bedroom, two-bathroom home. Children will have access to the daycare room, bedroom #1, kitchen, living room, and bathroom #2. Off-limits rooms and closets are made inaccessible by the use of doorknob spinners and a children’s safety gate.

There is a fireplace located in the living room near the kitchen. Applicant stated the fireplace will not be used during daycare hours. LPAs observed that the fireplace is barricaded with a children’s activity board secured with Velcro to prevent access. The home has central heating and air conditioning to ensure the safety and comfort of children in care.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPAs observed in the daycare room several children’s learning activities displayed on the walls, including numbers and days of the week, child-sized furniture, safe toys, arts and crafts materials, books, four cots, and two play yards. LPAs also observed child-sized furniture, safe toys, and books in bedroom #1. Applicant stated children will nap in bedroom #1 and the daycare room using cots, and infants will nap in play yards. Applicant understands children must be supervised at all times. LPAs provided applicant with the Individual Sleeping Plan and Safe Sleep handout.

The facility has a 2A10BC fire extinguisher mounted on the right-hand side wall of the home’s entryway. The parent board is located on the entryway wall. LPAs observed a functioning smoke alarm, carbon monoxide alarm, and first aid kit in place. Applicant’s Pediatric CPR and First Aid certification through the American Red Cross expires on 08/31/2026. Preventative Health and Safety training, including Nutrition and Lead Poisoning Prevention, was completed on 09/14/2024. Applicant completed the Mandated Reporter Training on 09/01/2024.

Knives are stored in a kitchen cabinet secured with a magnetic child safety lock and are inaccessible to children. Medications are stored in bathroom #1 and are inaccessible to children. Cleaning compounds are stored in the cabinet beneath the kitchen sink and in the laundry room, both of which are secured with magnetic child safety locks and doorknob spinners to prevent access by children.

Applicant was advised that fire drills must be conducted at least once every six months and documented with the date and time. A fire drill log was provided as an example. Applicant was advised that at least one staff member with current Pediatric CPR and Pediatric First Aid certification must be present at all times when children are in care.

LPAs observed no bodies of water on the premises. LPAs observed one fish in a covered tank located in the daycare room. Applicant was advised that she is responsible for ensuring the health and safety of children in care at all times, including supervision and safety related to pets and animals in the home.

Applicant stated there are no firearms, ammunition, or poisons in the home or on the premises. Applicant was reminded that all advertisements for daycare services, including business cards, flyers, posters, and signs, must include the facility license number in accordance with Title 22, California Code of Regulations, Section 102359(a), Advertisements and License Number. Applicant was further advised that failure to comply with applicable licensing laws and regulations may result in civil penalties and/or other enforcement action.

Applicant was advised that smoking is prohibited on the premises of a family child care home pursuant to Health and Safety Code Section 1596.795(a). Applicant stated the home is smoke-free. Applicant stated she will transport daycare children and understands that age-appropriate vehicle restraints and car seats must be used for all children being transported in accordance with applicable law.

The fenced backyard contains a children’s swing anchored to the ground, a sensory and play table, child-sized furniture, three ride-on toys, two scooters, safe toys, and a painting easel for children’s use. LPAs also observed two covered barbeque grills in the backyard. Applicant understands outdoor equipment and hazardous items must remain safe and inaccessible to children as appropriate.

LPAs discussed safe sleep regulations and provided applicant with Safe Sleep Regulation Concepts. Applicant was advised that it is her responsibility to read, understand, and maintain compliance with Title 22 regulations. Applicant was informed that regulations and licensing information are available through the Community Care Licensing Division (CCLD) website at www.ccld.ca.gov. SB 792 immunization requirements were verified and are on file.

Applicant was advised that the Fresno Regional Office of the Community Care Licensing Division has inspection authority and may inspect all areas of the home, including rooms, garages, and separate dwellings located on the premises, pursuant to applicable licensing laws and regulations.

Incidental Medical Services (IMS) policy was discussed. IMS are not currently being provided. Applicant was advised that if IMS are provided in the future, a Plan for Providing IMS must be submitted to the Department. Information regarding the Americans with Disabilities Act (ADA) was provided, including the U.S. Department of Justice (USDOJ) ADA Information Line at (800) 514-0301 (voice) and (800) 514-0383 (TTY).

Required postings and a forms packet, including the Parent Notification Requirements and LIC 9224, were provided and discussed. Applicant was advised that additional forms, updates, and provider resources are available on the Community Care Licensing Division website at www.ccld.ca.gov. Applicant was reminded of her responsibility to periodically review applicable laws and regulations. Applicant stated the daycare will operate Monday through Friday from 6:00 AM to 6:00 PM, and as arranged. No overnight care will be provided.

LPAs and applicant discussed the Community Care Licensing website, including the Provider Information Notification (PIN) system and Quarterly Updates, which provide information regarding new legislation, regulations, and provider requirements. Applicant was advised to visit http://www.cdss.ca.gov/, select “Information and Resources,” then “Community Care Licensing,” followed by “Quarterly Updates” and “Child Care Advocates Program” to register for PIN notifications.

Pending final review of the application file, licensure as a Large Family Day Care Home with a capacity of 14 children, ages under 18 years, will be recommended effective 05/13/2026.
