Licensing Program Analyst (LPA) Avelina Martinez arrived at the facility unannounced on 03/02/2022 at 1:00 PM to deliver complaint findings, LPA met with Theresa Pettapiece and explained the purpose of the visit.

Throughout the course of the investigation, LPA Martinez conducted interviews and reviewed facility records. Complaint 27-AS-20211101160116 has been completed, and it was determined this (27-AS-20211105081421) complaint shared the same allegations. The shared allegations are as follows: resident sustained injury while in care; incontinent resident was not kept clean and dry; dining room is unsanitary; resident was not provided with comfortable living accommodations. Therefore, there will be no additional deficiencies cited. Complaint 27-AS-20211101160116 citations can be found on the 12/28/2021 report.

Moreover, LPA Martinez and Pacifica Senior Living Modesto facility have discussed a plan of correction (POC). Pacifica Senior Living Modesto facility has submited the agreeded upon POC on 12/29/2021.

Licensing Program Analyst (LPA) Avelina Martinez arrived at the facility unannounced on 03/02/2022 at 1:00 pm to deliver complaint findings, LPA met with Theresa Pettapiece and explained the purpose of the visit.

Throughout the course of the investigation, LPA Martinez conducted interviews and reviewed facility records. LPA Martinez reviewed Covid-19 Screening (staff and Visitors) documentation for the month of October 2021. LPA Martinez reviewed and observed October 31, 2021 documentation. LPA observed witness 1 and witness 2 Covid-19 Screening documentation. LPA Martinez also crossed reference facility sign in sheet with Covid-19 documentation, and it was determined there was not a preponderance of evidence to prove that the facility was not screening visitors.

LPA Martinez conducted collateral interviews and reviewed medication documentation, and it was determined there was not enough evidence to prove that medications were not properly maintained. Also, LPA Martinez was not able to obtained or visually observed the medication. In addition, LPA Martinez was not able to observe Resident 1's (R1) clothing, and there was not substantial evidence to prove R1's clothing were missing.

Due to the above noted information, although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, and therefore the allegation ar unsubstantiated. Exit interview conducted, and a copy of this report was given to the facility.

As a result of this investigation, the Department finds these allegations to be substantiated. A finding that the complaint is substantiated means that the allegations are valid because the preponderance of the evidence standard has been met. An exit interview was conducted, and a copy of report provided to facility.
