Resident assaulted another resident due to lack of supervision - UNSUBSTANTIATED
During staff interviews it was learned that the resident’s doors were always locked and staff were always watching, therfore R2 could not have gone into R1’s room.
Administrator stated Staff were in R1’s room every 2 hours. When you put a hand on R1 they would scream and cry. It’s not possible that R1 would not have screamed if they were assaulted.
This allegation is unsubstantiated.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation occurred, and the findings are UNSUBSTANTIATED.
An exit interview was conducted. A copy of the report was provided to facility administrator Sue Todd.
