25 | On April 29, 2024 Licensing Program Analyst (LPA) Kari McWilliams conducted an unannounced case management visit and met with Director Kathryn Hardie. LPA McWilliams toured the facility and a census was taken. LPA McWilliams informed Director Hardie the purpose of this inspection was due to an incident.
On April 23, 2024, at approximately 8:30 a.m. the children were finishing breakfast, child #1 (C1) got up and was walking to go play when he self-tripped falling onto Child #2 (C2). C2 then fell forward causing to hit the legs of a child size wooden table. Staff #1 (S1) went to pick up C2 and noticed a cut approximately 2 inches across C2's forehead. Staff #2 was wearing gloves and went over and applied pressure.
S1 called Assistant Director Melissa Chavez who responded to the classroom. S1 also called C2 parent/guardians and both were at the facility in 7 minutes.
C2 was taken to Valley Children's Hospital where C2 received 9 stiches to the forehead. Assistant Director stated parents have opted to keep C2 home for the rest of the week for observations. LPA McWilliams confirmed C2 returned to the facility on April 29, 2024 with no restrictions.
Director Hardie confirmed that maintenance took the wooden table and has rounded the legs to prevent anyone else from hurting themselves. LPA McWilliams observed the table in the classroom and confirmed that the legs and side of the table have been rounded. LPA observed that the table is up against the wall and not in an area where children are running. LPA McWilliams observed that there were no hazards in the classroom.
Exit interview conducted and report was reviewed with Director Kathryn Hardie. Appeal rights were provided.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.
This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days. |
