25 | On 06/23/2026 Licensing Program Analyst (LPA), Anita Tristan, conducted an unannounced Case Management Inspection. A tour of the facility was completed and a census was conducted. LPA met with Director Lorena Lopez and explained the purpose of the inspection.
The Fresno South Regional Office received a report on 05/05/2026 regarding an incident that occurred on 05/04/2026 during nap time. Child #1 (C1) reportedly refused to lie down and subsequently threw themselves onto a cot, striking their chin on the metal frame located along the edge of the cot. The metal frame was covered by a fitted sheet and a mesh pad
Staff #1 (S1) observed that C1 sustained a laceration to the chin. Immediate first aid was provided, including cleaning the wound and applying ice to the affected area. The child’s parents were notified promptly and transported C1 to the emergency room for further medical evaluation.
During the inspection, LPA conducted interviews, reviewed facility records, and obtained photos relevant to the incident.
Based on the information gathered during the investigation, the incident appears to be isolated in nature. Facility staff responded appropriately to the injury by providing immediate care, notifying the child’s parents, and complying with appropriate policies, regulations, and reporting requirements.
Per California Code of Regulations Title 22, Division 12, no deficiencies are cited.
An exit interview was conducted with Director Lorena Lopez. The report was reviewed and appeal rights were provided and discussed.
Notice of Site Inspection shall remain posted for 30 days. This report shall be made available to the public upon request.
