On 11/16/2022, Licensing Program Analyst (LPA) Nancy Her conducted an unannounced case management inspection at the facility. LPA met with Preschool Coordinator Donna Hartman, to discuss an incident which occurred on 09/27/2022. LPA toured the facility inside and outside, took a census, and interviewed staff member.

Staff member stated during lunch time, around 11:00 AM, a child started choking on a piece of corn tortilla while eating a taco. The piece that the child bit was small enough that it could have slid down her throat without being chewed. It is believed that the child did not chew the food before swallowing. Staff member, who is First Aid/CPR certified, was sitting at the child’s table, asked the child if child was okay. The child kept coughing and did not respond, so the teacher did the Heimlich maneuver and the food flew out of the child's mouth. Child drank some water and said the child was fine. The child wanted to continue to finish the meal. The child did not sustain any injuries. This incident was reported to parents. Parents were not concerned and did not take the child to a physician.

During mealtimes, there is a staff member who sits with the children at the tables and supervises. The staff member talks to them, opens any items that need to be opened, and supports them. It is a “family style” meal except that meals are now already pre portioned and on individual trays for the students.

Based on the information obtained, this appears to be an isolated incident and facility took appropriate measures to address the incident, following proper policies and procedures and no regulations were violated. Preschool Coordinator reported the incident to Fresno Community Care Licensing on 10/06/2022.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.

Exit interview conducted and report was reviewed with the facility representative Donna Hartman.

A notice of site visit was given and must remain posted for 30 days.
