25 | On January 23, 2026, Licensing Program Analyst (LPA) Xona Xayavong conducted an unannounced case management–incident inspection. LPA met with Director Carmen Aguila, and a census was taken. LPA toured both the inside and outside of the facility. The purpose of today’s inspection was to follow up on an unusual incident report received by the Fresno South Regional Office on 12/5/2025.
On 12/5/2025, at approximately between 11:00 a.m. and 11:15 a.m., during outdoor activities on the playground, Child #1 (C1) approached Staff #1 (S1). S1 observed bark on C1’s left side, chest, and beanie. S1 also noticed C1 touching his arm and inspected him for any injuries. S1 disclosed that C1 did not display or report any pain or discomfort at that time. C1 was later picked up by an authorized representative, and staff informed the representative of the incident that occurred on the playground. Later at home, C1 displayed discomfort. C1’s mother took him to urgent care, and he was subsequently referred to Valley Children’s Hospital, where it was discovered that C1 had a broken elbow requiring a cast. Staff #4 (S4) spoke with C1’s mother on 12/5/2025 regarding the incident and again on 12/9/2025 to discuss restrictions at school to ensure C1’s safety, as the child now had a cast. C1 returned back to the facility on 1/12/2026 with an excuse from physical activities until 1/20/2026.
During today’s inspection, LPA conducted a review of staff and child records, staff interviews, observation and obtained facility documentation. After gathering information, LPA determined that staff took appropriate action and that the playground contained no hazardous items accessible to children in care.
An exit interview was conducted, and the report was reviewed with Director Carmen Aguila. Appeal rights were provided. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiencies are cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit was provided and is required to be posted for 30 days.
