25 | On 9/23/2022, Licensing Program Analyst (LPA) Theresa Marquez conducted a Case Management inspection and met with Director Teresa Castaneda. The purpose of this inspection is in regards to a self reported incident that occurred on 9/14/2022.
On 9/14/2022, Castaneda was in the nap room with 11 children. Teacher Mayra Rosas was also present in the center but had momentarily left the nap room. Castaneda was sitting with child #1 but walked away to tie the shoes of child #3. While Castaneda's back was turned she heard both child #1 and child #2 begin to cry. Castaneda then observed both children #1 and #2 hitting each other. Children #1 and #2 are both non-verbal.
Later that day, during outside playtime, child #1 and child #2 were sitting next to each other while eating their afternoon snacks. Castaneda again observed children #1 and #2 hitting each other. Castaneda then separated children #1 and #2 for the rest of the day. Staff did not notify parents of the 2 altercations because she did not observe injuries on neither child #1 or child #2.
On 9/15/2022, the authorized representative (AR) of child #1 arrived at the preschool to show cell phone pictures of injuries on child #1. The AR alleged that child #1 was bitten.
Castaneda stated neither child #1 or #2 have a history of biting, however child #1 has been exhibiting aggressive behavior. The authorized representative of child #1 terminated day care services on 9/15/2022.
LPA Marquez discussed with Castaneda supervision at the time of the incident. Castaneda was reminded that because child #1 is exhibiting aggressive behavior and children #1 and #2 were in a previous altercation earlier in the day, she and all staff need to be conscientious of providing adequate supervision and distance between fighting children. Castaneda was also reminded to inform parents of the children's daily activities specifically if the child is displaying aggressive behaviors.
Per Title 22, Division 12, Chapter 1 of the California Code of Regulations no deficiencies were observed today.
Exit interview was conducted with Director Teresa Castaneda. LPA provided Castaneda with a copy of the Facility Evaluation Report (LIC 809), and the Notice of Site Visit form (LIC 9213). The LIC 809 is required to remain in the facility for public review and the LIC 9213 is required to be posted for 30 days. |
