25 | On 11/25/2024, Licensing Program Analyst (LPA) Octavia Nolan conducted an unannounced Case Management Inspection at the facility. LPA met with Site Supervisor Twila Silva. LPA took a census and a tour of the facility.
The purpose of the visit was to inspect a newly installed playground structure at the facility. On 09/13/2024, Site Supervisor notified Community Care Licensing by telephone that construction began for two new playground structures.
One of the new play structures is in the outdoor activity area for the preschool. The play structure was professionally installed and is properly anchored into the ground. LPA observed the structure was installed appropriately and no hazards were observed. There were no loose or pointed parts. There is sand for cushioning surrounding the structure that adequately covers the fall zones. There is a large canopy above the play structure for shade. LPA reviewed the play structure manual and determined it is age-appropriate for children 2-5 years old. LPA requested that Site Supervisor obtain age-appropriate stickers from the manufacturer to affix to the structure. Site Supervisor was able to order stickers today.
LPA advised the Site Supervisor Twila Silva, effective immediately, the facility may use the new structure for preschool children.
Exit interview conducted and report was reviewed with Site Supervisor Twila Silva. Site Supervisor was provided appeal rights. This report shall be available to the public upon request. LIC 9213 Notice of Site Visit was provided and required to posted for 30 days.
