25 | On this date Licensing Program Analyst (LPA) Adrian Pizano and Licensing Program Manager (LPM) Luisa Gavoutian conducted an unannounced Case Management inspection. LPA and LPM met with Director Holly Oliviera, toured the facility, and took a census. The purpose of today’s inspection was to discuss an incident that was reported to Community Care Licensing (CCL) on 03/05/2024.
On 03/05/2024, at 10:35am, a toddler (Child 1) escaped from the facility via an unlocked gate on the side of the playground and was found in the middle of the street by the front neighbor.
The director, who directly witnessed this incident, stated that she noticed from the front classroom window a vehicle blocking the exit driveway. The director went out to investigate at 10:39am when she saw the front neighbor holding Child 1. Director stated that the side gate was left unlocked due to the handyman fixing the playhouses on the playground. Director viewed security footage from the incident and saw the child had been missing for 4 minutes. Director stated during the time of the incident there were 6 staff members and 44 children. The Porterville Police Department was contacted by the front neighbor. Director contacted Child 1 parent to notify them of the incident, then contacted Community Care Licensing (CCL) to report the incident.
LPA observed construction taking place on the West side of the playground that Director stated began on 03/05/2024. Director stated that children were using the playground on 03/05/2024 but have not been using it since. Three structures are being remodeled and painted. Children are currently using the smaller sectioned off play area underneath the covered patio. LPA also observed newly cemented areas on the South side of the playground. Director stated that the cemented areas were completed at the end of January 2024.
On 03/05/2024, at 6:00pm the director stated that a mandatory all staff meeting was held to discuss the severity of the incident. The Director stated that a staff member will always stand next to the gate during outdoor play time.
