On 10/17/2025, Licensing Program Analyst (LPA) Claribel Soto conducted an unannounced complaint inspection at the facility to deliver findings for the above-mentioned allegations. LPA met with Licensee, Karena Mattos and a census was taken. The purpose of today's inspection is to close the complaint investigation. During the course of the investigation, LPA interviewed staff, parents, children, reviewed and obtained facility records.

The allegation of day care child sustained multiple injuries due to lack of supervision was investigated and based off interviews and record review there is no indication that a child received an injury due to lack of supervision.

The allegation of licensee hit day care child was investigated and based off interviews there is no indication that licensee hit day care child.

This agency has investigated the complaint with the allegations and has been determined that the allegations are UNSUBSTANTIATED, meaning although the allegations may have happened or may be valid, there is not a preponderance of the evidence to prove the allegation stated above.

Per California Code of Regulations, Title 22, Division 12, Chapter 3, no deficiency is cited as it pertains to these allegations.

An exit interview was conducted with Licensee Karena Mattos. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days. This report shall be made available to the public upon request. Appeal Rights were provided.
