25 | On 09/23/2025, Licensing Program Analyst (LPA) Nohemi Sanchez conducted an unannounced case management inspection. LPA met with Emma Olivas, who accompanied LPA on a tour of the facility, both indoors and outdoors, and a census was taken.
During a complaint investigation, LPA Sanchez discovered through records reviewed and interviews conducted that an uncleared adult has been present and assisting at the facility, and on multiple occasions has been left alone to care for children in care. The licensee further admitted that the uncleared adult had assisted her on various occasions and, at times, has been left alone with children in care.
Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiency is being cited (see LIC809-D). Licensee, Emma Olivas was provided with a copy of their appeal rights.
LPA Nohemi Sanchez informed licensee Emma Olivas that this report dated 09/23/2025 document Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Nohemi Sanchez informed the licensee to provide a copy of this licensing report dated 09/23/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
