25 | On 03/19/25, Licensing Program Analyst (LPA) Denisia Jimenez arrived at the facility to conduct an unannounced Case Management inspection. LPA met with Pre-school Coordinator Carmen Aguila. LPA toured the facility, and a census was taken. The purpose of today's inspection was regarding an unusual incident that was reported to the Fresno Childcare Regional Office on 02/25/2025.
On 02/21/25 1 teacher and 2 preschool aides were outside on the playground area with 9 children. Teacher and 1 aide were supervising the play structure, and the other aide was supervising the grass area. Teacher and aide who were at the play structure heard a faint cry and observed Child #1 face down on the bark next to the stairs on the play structure. Staff stated that Child #1 missed a step while trying to climb up the steps on the play structure. Staff immediately acted and rendered aid to the child. Child #1 was bleeding from his mouth and staff cleaned its mouth. Staff contacted the site nurse who provided and ice pack and noticed child #1 tooth was loose. Site Nurse contacted parent who took child to the dentist and its tooth was extracted. Child #1 returned to the facility on 02/24/25 and is fine.
LPA inspected the playground structure and observed that the structure is safe and did not have any blind spots for lack of supervision. LPA confirmed that the playground has soft cushion floor for all high climbing areas to absorb falls.
Based on the information obtained, this appears to be an isolated incident, and staff took appropriate measures to address the incident and followed appropriate reporting requirements.
Per Title 22, Division 12, of the California Code of Regulations, no deficiencies are cited.
