On 06/09/2026, Licensing Program Analyst (LPA) Sonja Navarrette conducted an unannounced complaint inspection to deliver findings. LPA met with Site Supervisor Valissa Elvankeyis Estrada, who accompanied LPA on a tour of the facility, both inside and outside. LPA explained and discussed the allegation and conducted a census. LPA interviewed witnesses, reviewed sign in/out sheets, and reviewed facility records.

During the investigation, witnesses disclosed that both children and adults have tripped or fallen due to uneven cement walkways and tree roots that significantly protrude from the ground in the preschool play yard. LPA reviewed children's files and ouch reports, which documented 10 incidents directly related to the protruding roots and uneven cement. LPA interviewed the Director, who confirmed awareness of the preschool play yard’s condition since 2025, and safety measures for preschool children had not been implemented.

Based on observations and information obtained through interviews, the preponderance of evidence standard has been met. Therefore, the above allegation is substantiated.

Per California Code of Regulations, Title 22, Division 12, the following deficiency was cited: (see 9099-D). An exit interview was conducted with Site Supervisor Valissa Elvankeyis Estrada. Appeal Rights were provided today. LIC 9213 Notice of Site Visit was provided and will be posted for 30 days.

The licensee reported they will submit a safety plan for preschool play yard to LPA by POC due date. Safety plan to include children safety, time frames and updates on removal or repair of cement and tree roots.
