25 | On 3/15/2024, Licensing Program Analyst (LPA) Theresa Marquez conducted a Case Management inspection and met with Teacher Debbie Arce. The purpose of this inspection is to discuss the preschools responsibility to report unusual incidents to Community Care Licensing (CCL).
In December 2023, a child climbed out of a plastic car through the window causing the car to tip over. The child fell onto the cement, cut his chin, and required stiches.
Staff did not report this incident in a timely manner to CCL and did not to submit the Unusual Incident report form LIC624 timely.
In February 2024, Director Finerty contacted the Officer of the Day to report the December 2023 incident and faxed form LIC624 Unusual Incident report to the CCL office.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: Staff failed to report an unusual incident in a timely manner. (see page, LIC809-D).
Teacher Arce contacted Director Kimberly Finerty to review this report. Finerty authorized Debbie Arce to sign this document. LPA provided Arce with a copy of the Facility Evaluation Report (LIC 809), the Notice of Site Visit form (LIC 9213) and the LIcensee Appeals Rights. The Notice of Site Visit is required to be posted for 30 days.
