25 | On 09/22/2026, Licensing Program Analyst (LPA), Sonja Navarrette conducted an unannounced Case Management inspection. The purpose of the inspection was to follow up on an unusual incident report (UIR) that was reported to the Department on 09/11/2026.
Today, LPA met with Director Korissa Brown, toured the facility, and conducted a census of the children in care. The LPA interviewed staff and reviewed children’s files.
According to the incident report, Child #1, who has existing health issues, awoke from a nap limping. The mother was contacted immediately and transported the child for medical evaluation. An X-ray indicated a possible fractured bone in the child’s leg.
Based on record review, the mother manages the child’s medication at home and provides daily updates to facility staff regarding the child’s condition. Based on staff interviews and record review, staff are aware of the child’s medical needs, which are monitored and documented each day at the child’s day care center. Staff are trained to identify any health concerns and to promptly notify the child’s mother. The LPA and the director discussed liability considerations and emphasized the importance of maintaining detailed documentation.
Based on the information obtained, this appears to be an isolated incident, and staff members took appropriate measures to address the child's medical condition, following proper policies and procedures and no regulations were violated.
Per California Code of Regulations, Title 22, Division 12, no deficiency cited. Exit interview conducted and report was reviewed with Director Korissa Brown. A notice of site visit was given and must remain posted for 30 days.
