On August 24, 2026, Licensing Program Analysts (LPAs) Aurelio Mendoza and Xona Xayavong conducted an announced case management inspection initiated by Preschool Director Yerania De Leon for a requested change in preschool capacity. Upon arrival, the LPAs met with Preschool Director Yerania De Leon.

The applicant is requesting to increase the licensed preschool capacity to 96 children, ages 2 years to 6 years, to be housed in classrooms 52, 80, 81, and 82. The previously approved capacity for these rooms was 72 children. Preschool Director Yerania De Leon stated that the program intends to convert to a full-day preschool operation. The LPAs reminded Preschool Director Yerania De Leon that any changes involving classrooms, capacity, or use of space must be submitted to and approved by the Fresno Child Care Licensing office prior to implementation.

The LPAs inspected classrooms 52, 80, 81, and 82. All rooms were observed to be clean, orderly, and free of toxins. Furniture and equipment were safe, age appropriate, and in good condition. Shelving units were properly anchored to prevent tipping hazards. Each classroom contained sufficient tables, chairs, cubbies, mats, and toys for preschool children. Smoke detectors and carbon monoxide detectors were installed in the central ceiling areas of each classroom and appeared to be functioning properly.

Drinking water is readily accessible indoors through drinking fountains, a water dispenser with disposable cups, and children’s personal water bottles. The facility provides breakfast and lunch, which are delivered from the central district office’s kitchen. Parents may also provide food from home.

The LPAs reminded Preschool Director Yerania De Leon that classroom furniture must be arranged to prevent tipping hazards and to ensure full visual supervision of children at all times.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Indoor measurements previously taken by LPA Nancy Her on July 10, 2025, established that classrooms 52, 81, and 82 measured a combined total of 2,540 square feet. During today’s inspection, LPAs Mendoza and Xayavong measured classroom 80 and determined that it totals 897 square feet, including 24 square feet of encumbered space. After deducting encumbered space, the usable square footage of classroom 80 is 873 square feet.

The total usable indoor square footage for classrooms 52, 80, 81, and 82 is 3,413 square feet. Adequate indoor storage space for children’s belongings was observed. The LPAs reminded Preschool Director Yerania De Leon that children’s equipment must match the licensed capacity.

The LPAs observed seven toilets, six sinks, and four urinals available for preschool children. These restrooms are located adjacent to classroom 52. A staff restroom is located in the school’s main office. The designated isolation area for ill children is located in the nurse’s office in the main office.

The preschool outdoor area is fully fenced with fencing at least four feet high. The total outdoor activity space measures 9,324 square feet, which is sufficient for the requested capacity of 96 children. Shade is provided by trees and umbrellas. The playground contains age-appropriate toys and equipment, including two climbing structures designed for children ages 2 to 5 years, both of which are properly anchored. Adequate cushioning within fall zones is provided by rubber pour surfacing. Drinking water is available through a water fountain. All hazardous items on the playground were observed to be inaccessible to children.

The LPAs reminded Preschool Director Yerania De Leon that any operational or structural changes to the preschool outdoor area must be reported to Child Care Licensing for review and approval.

A fire clearance approving a capacity of 96 preschool children was received on August 20, 2026, from the Tulare County Fire Department.

The LPAs reminded the Facility Representative that all adults age 18 and older who supervise, provide care, or have contact with children, except as otherwise permitted under Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption prior to being present in the facility. Failure to comply will result in a civil penalty of $100 per day, up to five days, or up to thirty days for repeat violations.

Preschool Director Yerania De Leon stated that the preschool program provides Incidental Medical Services (IMS). The LPAs provided IMS guidance, including information from PIN 22-02-CCP. If IMS is provided, the facility must submit an updated Plan of Operation that includes IMS. The LPAs also provided resources related to the Americans with Disabilities Act (ADA), including contact information for the Department of Justice ADA Information Line and the publication “Commonly Asked Questions About Child Care Centers and the ADA.”

The LPAs reviewed LIC 311A with Preschool Director Yerania De Leon, which includes preschool child records, personnel records, administrative records, and required postings. The LPAs also provided information regarding the MyChildCarePlan.org website, which connects families to child care providers and Resource and Referral Agencies. Preschool Director Yerania De Leon was reminded to subscribe to Provider Information Notices (PINs), Quarterly Updates, and other important communications available through the Community Care Licensing Division website.

The shared restroom waiver and the playground waiver are pending review and approval. Both waiver requests were submitted during today’s inspection. The Preschool Director was informed that the waiver requests must be reviewed by Child Care Licensing Management before approval.

A license for 96 preschool children will be issued once the final file review is completed and the shared restroom waiver and playground waiver have been approved.

No deficiencies were cited during today’s inspection, in accordance with Title 22, Division 12, Chapter 1 of the California Code of Regulations. An exit interview was conducted, and this report was reviewed with Preschool Director Yerania De Leon. A Notice of Site Visit was provided and must remain posted for 30 days.
