On 08/11/2022 Licensing Program Analyst (LPA) Nancy Her, conducted an unannounced Case Management Inspection and was met by Licensee Cheryl Mendiola and took a census. Also present were Staff #1 and #2. The purpose of today’s inspection is to clear the citation that was cited on 07/21/2022 with a POC date of 07/29/2022:

The northwest gate is not five feet high. The openings in between each barrier exceeds 4 inches in diameter.

During today’s inspection, LPA measured the gate. The gate now meets the Division 1, Appendix Chapter 4 of the 1994 Uniform Building Code requirements.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are cited.

Exit interview conducted and report was reviewed with the facility representative Cheryl Mendiola.

A notice of site visit was given and must remain posted for 30 days.
