On July 15, 2026, Licensing Program Analyst (LPA) Xona Xayavong conducted an announced Case Management inspection - licensee initiated for an increase in capacity. Upon arrival, LPA met with Facility Representative Bianca Rodriguez. The applicant is requesting to be licensed for 75 preschool children ages 2 years to 6 years in Classrooms A2, A3, and A4. The previous capacity was approved for 24 preschool children ages 2 to 6 years old in Classroom 16 & 17. However, Facility Representative stated Classroom 16 and 17, the shared outdoor activity space with the transition kindergarten, and restroom north of Classroom 16 will no longer be utilized. The applicant is aware that any changes in classrooms and/or capacity must be reported to the Fresno Community Care Licensing office.

Classrooms A2, A3, and A4 were observed to be clean and free of toxins. LPA observed that the furniture and equipment appeared to be safe and in good condition. The shelving units are anchored to prevent tipping onto children in care. There are enough tables, cubbies, chairs, mats, and ample toys for children to play with. Working smoke detectors and carbon monoxide detectors were observed in the classrooms, installed on the ceiling.

Drinking water is available both indoors and outdoors and is provided by drinking fountains and water dispensers with disposable cups. LPA confirmed that the building construction was completed on June 23, 2026. The facility provides lunch and snacks, which are prepared off site.



LPA reviewed with the Facility Representative that any future changes to the classrooms must ensure that furniture does not pose a tipping hazard, and furniture must be anchored or arranged to prevent tipping. Classroom furniture arrangements must allow for 100% visual supervision of children at all times. (Continue on LIC 809-C)

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA measured classrooms A2 (1,031 sq. ft.), A3 (1,031 sq. ft.), and A4 (1,031 sq. ft.), resulting in a total of 3,093 square feet, with a deduction of 116 square feet for encumbered spaces such as cabinets and changing tables. The total indoor preschool area measured approximately 2,977 square feet, which accommodates a capacity of 75 preschool children at one time. There is adequate storage space available for children’s belongings. Facility Representative understands that children’s equipment must match the licensed capacity. Toys and equipment appear to be age-appropriate.

The facility has exclusive use of two toilets and one sink inside each classroom bathroom in classrooms A2, A3, and A4. LPA also observed two sinks (one adult and one child) near the changing table in classrooms A2, A3, and A4. There is a separate staff restroom equipped with a toilet and a sink located in classroom B1. The isolation area for children who become ill will be in the breakroom/site supervisor’s office between classrooms A1 and A2.

The facility currently has a fully fenced playground area. The chain-link fencing is at least four feet high. The total square footage for all outdoor activity space is 5,710 square feet, which is sufficient to accommodate the requested capacity of 75. Shade is provided by two temporary metal canopies. There are sufficient age-appropriate outdoor toys and play equipment available. The Facility Representative stated that the applicant plans to install a permanent shade structure. The applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

A fire clearance for 75 preschool children was received on July 8, 2026, and approved by the Farmersville Fire Department.

The Facility Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff; persons who provide care and supervision to children; and staff or volunteers who have contact with children, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of a minimum of $100.00 per day, for up to 5 days—or for up to 30 days for repeat violations—will be assessed if this regulation is violated.

LPA reviewed with the Facility Representative the LIC 311A, Records to Be Maintained at the Facility, including child records, personnel records, administrative records, and required postings.

The applicant was informed of the MyChildCarePlan.org website, a consumer education resource that helps families obtain child care by connecting them with providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders through Provider Information Notices (PINs), Program Quarterly Update newsletters, and other communication platforms. To receive important child care licensing information, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email updates.

No corrections are needed. A license for 75 children will be granted upon final file review.

Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiencies were cited. An exit interview was conducted, and the report was reviewed with Facility Representative Bianca Rodriguez. A Notice of Site Visit was provided and must remain posted for 30 days.
