25 | On 09/17/2026, Licensing Program Analyst (LPA) Aurelio Mendoza met with Site Supervisor Lilia Perez and conducted an unannounced case management–incident inspection. LPA explained the purpose of the inspection and toured the indoor and outdoor areas of the facility. A census was taken.
This inspection followed an unusual incident self-reported to the Fresno Regional Office on 08/25/2026. Records and interviews indicated that on 08/24/2026 at approximately 9:15 a.m., a preschool child tripped while attempting to climb into a metal fire truck play structure during outdoor play, resulting in the child striking their mouth and chin.
At the time of the incident, four staff members (two teachers and two teacher assistants) were supervising 27 children. Staff provided immediate first aid, including cleaning the injury and applying ice. The authorized representative was notified promptly. The child was signed out at 10:12 a.m., picked up by the authorized representative, and taken for dental evaluation and treatment.
Records reviewed indicated that dental instructions included monitoring the child while eating and that the child could return to care on 08/26/2026 with restrictions requiring the child to eat soft foods or foods cut into small pieces for two weeks. Staff reported today that the facility adhered to these restrictions. The child returned to care on 08/27/2026 and was observed to be present during today’s inspection.
LPA observed that the fire truck play structure involved in the incident had been removed from the playground and stored indoors in an inaccessible area. The unit appeared age-appropriate, metal, in good repair, and free of defects.
Based on interviews, observations, and record review, LPA determined that staff responded appropriately, adequate supervision was present, and the incident was isolated and not caused by faulty equipment.
An exit interview was conducted with Site Supervisor Lilia Perez. Appeal rights were provided. No deficiencies were cited. LIC 9213 Notice of Site Visit was issued and must remain posted for 30 days.
