An unannounced Complaint visit was conducted on the date & times indicated above by Licensing Program Analyst (LPA) K. Mcclurg. LPA met with Assistant Administrator (AA) Regan Lockett & stated purpose of visit.

The above allegation was investigated by law enforcement. Law enforcement did not confirm &/or corroborate that abuse took place.
The Department has investigated the above allegation & determined it to be Unsubstantiated.
