On January 27, 2026, at 10:30AM Licensing Program Analysts(LPAs) Deborah Khashe and Sarah Tibbett conducted an unannounced visit to deliver findings for the above allegation. LPAs met with the Director Ali Cardoza. LPAs disclosed the purpose of the inspection. LPAs toured the facility and observed twenty (20) childcare children being supervisor by four(4) childcare teachers.
It was alleged that staff left day care child in soiled diaper. During the investigation LPA Khashe toured the facility, observed staff interactions with and supervision of children in care, conducted interviews with teachers, parents, director and reporting party, and obtained pertinent documents. The reporting party stated when the child was picked up from day care the child’s pants were wet in the diaper region and the diaper was so full it was bursting at the seams. The reporting party stated that the child had a very red rash and her child had blisters and bleeding. Parent checked the ProCare app and said she had not been changed all day. Mom reported that the child was dropped off at 9:28AM and picked up at 4:30PM.

During the investigation, LPA Khashe read email exchanges between the complainant and Ali Cardoza that admit to a teacher making a mistake and that the child in question went home in a condition that was unacceptable by Safari’s standards, policies or level of care.
Based on corroborating information from interviews conducted, there is a preponderance of evidence to prove the alleged violation did occur at the facility. Therefore, the above allegation is found to be SUBSTANTIATED.

ALi Cardoza will have a staff meeting/ training and review diaper changing protocals and proceedure. Ali Cardoza will submit a sign copy to LPA Khashe that is signed by all staff that attended. It will include dated with agenda and information covered with staff. This Proof of Correction can be emailed.
