13 | On 7/14/2026, Licensing Program Analyst (LPA) Sarah Tibbett met with Licensee, Katie Pack, to deliver the findings of the complaint investigation regarding the above allegations.
During the course of the investigation, LPA Tibbett conducted interviews, made observations, reviewed files, and obtained information pertaining to the allegations. It was alleged that staff restrained a child and interfered with a child eating their lunch. Through interviews, it was revealed that staff observed a potential choking hazard with a large food item in a child’s mouth. A staff member approached the child and removed the food from their mouth. Consistent information from interviews determined that the child was not restrained during the episode and that the child continued eating their lunch after the incident.
Based on information obtained, the above allegations could not be substantiated or dismissed. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the findings are UNSUBSTANTIATED. A notice of site visit was provided and must remain posted for 30 days.
