On October 22, 2024 at 11:20 AM, Licensing Program Analyst (LPA) Aaliyah Zendejas made an unannounced visit for the purpose of conducting a Case Management - Incident inspection. LPA met with Director Peggy Schreiber to discuss an incident that was self reported to Community Care Licensing Division (CCLD) office by telephone call on September 25, 2024. LPA toured the play yard area where the incident occurred and interviewed the Director, and staff that were involved in the incident.

On 09/25/2024 an incident occurred at the center at about 10:00 AM. S1 saw the child fall from the play structure after trying to reach a pole attached to the play structure. S1 states they saw child not be able to reach the pole and fall on their bottom. S1 stated that C1 took a second to catch their breath then they started to cry. Staff called the parent of the child who later took them to urgent care and was told the child had some bruising on their hip and leg but would be fine.

Child returned to center the next day. To prevent any similar incidents, the center had installed plexiglass the following weekend on September 28. Center stated that they had extra supervision around that area and did not allow any children to go on the upper level for the few days before the plexiglass was installed. LPA viewed playground area and did not see any other hazards related to the incident that could occur due to the other areas not being at a bigger height and also due to the installation of the plexiglass.

Given the incident was observed by staff who took appropriate action by immediately reporting to licensing and the parent of the child as well as implementing changes to ensure that children no longer have any incidents on the playground structure no deficiencies are being cited.

A Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days.
Appeal Rights were provided to Director Peggy Schreiber.
Exit interview conducted and report was reviewed with the Director Peggy Schrieber.
