25 | On October 22, 2024 at 12:56 PM , Licensing Program Analyst (LPA) Aaliyah Zendejas made an unannounced visit for the purpose of conducting a Case Management - Incident inspection. LPA met with Director Peggy Schreiber to discuss an incident that was self reported to Community Care Licensing Division (CCLD) office by telephone call on October 17, 2024. LPA toured the yard area where the incident occurred and interviewed the Director and staff involved in the incident. LPA also reviewed the security tapes of the incident that occurred.
On 10/16/2024 from 3:37 PM until 4:11 PM, a child was left unattended in the enclosed outdoor yard. S1 was bringing the children inside from being in the outdoor area. Director states that children are lined up outside and then brought inside. On 10/16/2024, S1 did not do their count before bringing children inside resulting in one child being left outside. After around 30 minutes, S2 went outside due to the grandfather of C1 coming to pick up the child and staff realized that child was not inside. S2 saw that child was found sitting, and frustrated because they were trying to get their shoes on.
Child returned to center the next day. To prevent any similar incidents, the center has implemented a physical roster that is used to keep track of children, a staff member doing a final sweep of the outdoor area, and a meeting that was held the day after the incident and today 10/22/2024 to ensure staff is aware of the new policies.
Given the child was left unattended for 30 minutes, and staff did not realize child was left unattended until someone came to pick them up, one Type B deficiency is being cited today. See 809-D for details.
